Application List

To begin the process of applying for a Water and Waste Direct Loan and/or Grant, complete and submit the following documents to your local Rural Development Office.  Please use this as a checklist and submit with the listed applications documents:

• Public Information Requirements  (publish at least 10 days prior to the meeting)
  Minutes from the meeting also to be submitted with application
• Resolution of Applicant's Board Authorizing Application
• SF 424, Application for Federal Assistance
    -Instructions for Obtaining a DUNS #
• sam.gov   Registration is required for all applicants applying for federal assistance.
• SF 424D, Assurances
• Organizational Documents/Articles of Incorporation and By-laws, including all amendments (Not-for-Profit Organizations Only)
• For public bodies, contact your local Rural Development Office for guidance on documentation of organization and legal authority
• Preliminary Engineering Report, completed according to the listed bulletins (please submit two copies)
  • Bulletin 1780-2 for water projects
  • Bulletin 1780-3 for sewer projects
• Environmental Assessment  (Prepared by Engineer or Applicant Representative)
  (Early submittal recommended.  Funding cannot be determined until environmental review is complete.)

• Most current Fiscal Year's Audit or Year-End Financial Report with Balance Sheet and Income Statement (forms RD 442-2 and 442-3 or similar format may be used)
• Current Fiscal Year's Operating Budget for Enterprise (form 442-2 or similar format may be used)
• IA Guide 13j, Water/Sewer Project Information
• IA Guide 13h, Certification of Existing Non-RD Indebtedness
• Form RD 400-1, Equal Opportunity Agreement
• Form RD 400-4, Assurance Agreement
• AD 1047, Certification Regarding Debarment, Suspension, and Other Responsibility Matters
• AD 1049, Certification Regarding Drug-Free Workplace Requirements
• RD 1940-Q, Exhibit A, Certification for Contracts, Grants, and Loans
• Form RD 1910-11, Applicant Certification Federal Collection Policies
• Survey on Ensuring Equal Opportunity for Applicants (optional)
• Representation Regarding Felony Conviction or Tax Delinquent Status for Corporate Applicants
