Forms for Grants -
Business Programs
The links below provide access to commonly-required forms in Rural
Development grant programs. Not all forms are required in all cases, so please
consult with Rural Development staff.
Does the business borrower have DUNS
number?
All RD grant and guaranteed loan applicants, except individuals &
informal steering committees, must have a Dun & Bradstreet (D&B)
Data Universal Numbering Systems number ("DUNS number") when
applying.
A DUNS number is a
requirement for virtually all applicants for Federal financial
assistance, including businesses receiving a commercial loan with a
Federal guarantee.
If you already have a DUNS number, you don't need to get a new one.
To obtain this number, call D&B Federal Registration Department
toll-free at 1-866-705-5711 and ask for a DUNS number.
It free & takes only 5 minutes. A D&B agent will ask for some
basic information and then assign you a DUNS number over the phone.
You may also obtain a DUNS number online --
http://fedgov.dnb.com/webform -- but we do not recommend this
because we have heard of instances where parties have had to pay to
get their DUNS number.
Are you registered on the Central
Contractor Registration (CCR)?
All RD grant applicants, except individuals & informal steering
committees, must be registered in the Central Contractor
Registration (CCR). CCR registration is a
requirement for virtually all parties seeking Federal financial
assistance, not just government contractors.
In order to register, you will first need a DUNS number (see above).
To complete a CCR registration, go to http://www.ccr.gov, create an
account (ID and password), and provide the required information
(including DUNS#, Tax ID#, statistical information about your
business, and Electronic Funds Transfer Information for payments).
It takes 1-3 days for CCR registration to take effect, so please do
not put this off. If your are already registered in CCR, be sure
your information is current and accurate. If you receive an RD
award, you must keep your CCR information current, updating it at
least annually, until your award project is complete.
