Community Facilities Grant Program

To begin the process of applying for a Community Facilities grant, complete and submit the following documents to your local Rural Development Office:
 Pre-Application Documents
o     SF 424, Pre-application for Federal Assistance
                 -Instructions for Obtaining a DUNS #
o     Applicant Narrative
o     Balance Sheets and Income Statements or Audits for past 3 years (Forms RD 442-2 and RD 442-3 or similar format may be used)
o     Current Fiscal Year's Operating Budget (Form RD 442-2 or similar format may be used)
o     Other Credit Certification
o     Evidence of Community Support (non-profits only)
o     Organizational Documents
                 -Articles of Incorporation and By-laws (non-profits only)
                 -Letter from Organization's Attorney (public bodies only)
o     Survey on Ensuring Equal Opportunity for Applicants optional

Upon receipt of a complete pre-application package, your local Rural Development Office will determine applicant and project eligibility, the level of environmental information needed, and if any additional financial information is needed. You will be notified of the agency's decision within 45 days and given further instructions on needed documents. Please be advised that you are required to hold a public meeting prior to grant approval. Instructions for meeting the public information requirements are found below. At a minimum, you will also need to provide the following documents to complete an application:

o     SF 424B, Assurances (non-construction) or SF 424D, Assurances (construction)
o     Resolution of Governing Board Authorizing Application
o     Published Notice of Public Meeting/Minutes of Meeting
o     Form RD 1940-20, Request for Environmental Information
o     Preliminary Architecture/Engineering Report with itemized cost estimate (construction projects only)
o     Form RD 400-1, Equal Opportunity Agreement (construction projects only)
o     Form RD 400-4, Assurance Agreement
o     AD 1047, Certification Regarding Debarment, Suspension, and Other Responsibility Matters
o     AD 1049, Certification Regarding Drug-Free Workplace Requirements
o     RD 1940-Q, Exhibit A, Certification for Contracts, Grants, and Loans
o     Form RD 442-7, Operating Budget (for proposed facility)

 Other Documents that May be Needed
o     Architectural Services Agreement
o     Legal Service Agreement - RD IA 1942-37 (non-profits only) or RD IA 1942-37b (public bodies only)
