Forms for Guaranteed Lenders (B&I and REAP)

The links below provide access to commonly-required forms in RD guaranteed loan programs.  Not all forms are required in all cases, so please consult with RD staff.
 
Does the business borrower have a DUNS number?

All RD grant and guaranteed loan applicants, except individuals & informal steering committees, must have a Dun & Bradstreet (D&B) Data Universal Numbering Systems number ("DUNS number") when applying.  A DUNS number is a requirement for virtually all applicants for Federal financial assistance, including businesses receiving a commercial loan with a Federal guarantee.

If you already have a DUNS number, you don't need to get a new one.  To obtain this number, call D&B’s Federal Registration Department toll-free at 1-866-705-5711 and ask for a DUNS number.  It’s free & takes only 5 minutes.  A D&B agent will ask for some basic information and then assign you a DUNS number over the phone.  You may also obtain a DUNS number online -- http://fedgov.dnb.com/webform -- but we do not recommend this because we have heard of instances where parties have had to pay to get their DUNS number.
