Tips on Filling Out the Forms



The SF-424 - Application for Federal Assistance for the Value-Added Development Grant

Block 1: The Type of submission is Applications, Non-Construction

Block 2: Applicant identifier may be left blank, or you may use what ever accounting identifier you wish.

Blocks 3 & 4: Leave blank

Block 5: Legal Name is the name that the grant checks would be made out to. Organizational Unit may be blank, or if a particular department or unit is responsible for the grant application that unit's name could be included here.

Block 6: If individual applicant then Social Security Number is OK. If EIN number is not yet issued, indicate such and provide that number before any grant money can be awarded.

Block 7: Most applicants will be L. or M., but could be N. - Other.

Block 8: New

Block 9: USDA, Rural Business-Cooperative Service

Block 10: Catalog of Federal Domestic Assistance Number: 10.352

Title: VADG

Block 11: Your descriptive title so that we can track your proposal versus the others received.

Block 12: Usually just where the processing, or value-adding activity is taking place.

Block 13: Start Date must be after the grant is awarded, likely after October1, 2002.

Ending Date should be reasonable but, before October 1, 2006

Block 14: a. Applicant - the congressional district of the organization or individual applying

b. Project - the congressional districts of all the producers and plants involved in the project.

Block 15: a. Federal - the amount requested from USDA for the entire project.

b. Applicant - the amount the independent producer applicant is providing, including in-kind.

c. State - any amount going to the project from State grants or loans, including in-kind assistance.

d. Local - any amount, including in-kind, going to the project from local government agencies.

e. Other - any amount going to the project from partners or other interests not listed above.

f. Program Income - any sales or other income that will be generated by the project.

Block 16: Only State Agencies are subject to review by the State Executive Order 12372 process; therefore, most applicants would check the No, Program is not covered by E.O. 12372.

Block 17: FSA and other loans must be considered when answering this question.

Block 18: Authorized representative is usually the applicant if an individual or the CEO, chairman, or some other officer if the applicant is a cooperative, corporation, or LLC.

Line 6 Break out the numbers from line 1 into the different types of expenses. Note that indirect charges are not allowed in for-profit organizations or individuals.
